Ministry Formation Center forms cannot be downloaded from this website. Catechist Certification: Basic - Evaluation Forms
The evaluation forms listed below are to be given to an already-certified catechist whose responsibility it will be to evaluate the candidate seeking certification. Ideally, this evaluation is conducted by the parish Director/Coordinator of Religious Education, or someone trusted and delegated by them to conduct the evaluation.

The observation forms below are for use by the candidate seeking certification as a catechist. They are to be used by the candidate to document their observation of an already-certified catechist engaged in catechetical ministry.

Please note that Recertification forms are processed on the fourth (4th) Friday of each month except for December when they are processed on or before the third (3rd) Friday.
